Live Well Community Health Centers of Waverly and Concordia
A new model for rural health care

Rural health care is unique. That is why the Live Well Community Health Centers of Waverly and Concordia have assumed a different approach in treating patients. With the help of our partners, like Pathways, Lafayette Regional Health Center, I-70 Medical Center, Fitzgibbon Hospital and Carroll County Memorial Hospital, among others, we take a collaborative approach to treat the whole patient.  Our Live Well Centers operate using a "health home" model which allows those we serve to come to one place to get their needs met.

We also believe in preventive health care. This means helping our patients identify and address health risk factors before they become health problems. This includes conditions like diabetes, obesity and tobacco use. Our goal is to provide patients with the treatment they need to live life well, while equipping them with tools they need to avoid preventable disease.
